Dr. Ouellette’s Wellness RISK Management Progress
Chart
And
Understanding Your Wellness RISK
Management Progress Chart Printout
This is a redacted progress chart from a Body Builder in Dr. Ouellette's practice. He
was quite fit with a lot of muscle mass. (Redacted means we have erased the codename.)
Legend
Red
background in a table cell below indicates a Self-Help
non-Therapeutic priority level. Whereas a yellow background indicates a secondary priority.
Caution here please: The Red and
Yellow cell backgrounds do NOT indicate
RED and YELLOW FLAGS. Flags are a clinical skill set for clinicians trained in the
meaning of Red and Yellow Flags and how to evaluate them. You will notice that in the
clinical Module of the chart below there is a Red Flags item and a Yellow Flags item. This program
evaluates for these FLAGS. Those items DO indicate clinical caution and
possible further evaluation. That is a clinical call for registered healthcare professionals.
The patient/client/participant reviewing this type of a Wellness
RISK Management Progress Chart document will likely
have questions on their scores in these FLAG categories due to the significance of these
items. We will not get into that here. It is not the place of the Wellness
RISK Management Instructor to offer advice on these
issues. Rather, a registered healthcare professional with privileges to diagnose, is the place to go for that
type of advice. So, be thinking Medical Doctors or Chiropractic Doctors.
Also, the Body Mass Index on Body Builders presents an error due to the extra muscle mass.
It is up to the healthcare professional to adjust for that discrepancy.

Ouellette’s Wellness RISK Measurement https://WellnessRISKManagement.com
Understanding Your Wellness RISK
Management Progress Chart Printout
Wellness RISK Management is designed for people to measure themselves online then manage their
own Wellness strategies.
Based on their scores they select their chores.
First, people need to learn how.
Your Progress Chart has five sections to it.
- Your five lifestyle measurements
- A more thorough nutritional assessment
- A set of important clinical assessments
- A Pain Relief Diet assessment
- A group of averages for statistical assessment
The first time you take the measurement questionnaire you will have an instructor helping you learn how to do it
on your own. Thereafter, you can do the first four pages to focus on your Action Plan items only, and then
check off one item only (usually the first item) on each page after that so that we know you intentionally saw
these pages. Some people may want to complete the last page as well to get an idea if food is impacting their
Wellness. That is your choice. Some other people may want to do the whole Pain
Relief Diet module.
The scores on those subsequent pages (Page Numbers 5-11) will show that you did either very well (95-100%) or
very poorly (0-1%). We will see this profile and will know that you deliberately didn’t fill out the pages to save
time. Your first set of scores is always kept as a baseline for future reference. That is why you do the entire 11
pages carefully on your first measurement.
The sixth time you take the questionnaire the second column of scores will be deleted, and each column will be
moved over once with the last column always being the most current column. Comparisons can then be made between the
last four columns and also with your starting point. The timeline date is posted at the top of each column for easy
reference. Should you want a COMPLETE assessment chart, just fill out all pages completely when you take it
again and be mindful of the breaks suggested on the pages.
If you want to send this progress chart to someone, then send the Redacted Chart either by
email or by printing a hard copy and having someone else print your name on the chart.
If you lose your Redacted Chart copy then you have two options.
1. The Free Option
-
- Print the first page of your chart.
- Blackout your codename with a magic-marker.
- Photocopy the final sheet.
- Have someone else print your real name beside the blackout.
- Provide that last photocopy to whomever may want it as either a paper copy or
you can convert it into electronic form by scanning, then send it as an attachment.
2. Pay a Fee Option
-
- Pay Wellness RISK
Management Head Office a fee and get a new Redacted copy from
Wellness RISK Management Head Office. (Best idea is to create a
Wellness RISK Management folder for yourself off your 'C drive' and store
your Progress Charts in there.)
Beginning a Wellness RISK Management Improvement Program
MEASURING and MANAGING Wellness
RISK Online
There isn't any magic in any of the healthcare fields. There are no gods one can call on to provide Wellness to
us. There are no meditations one can do that magically boost your Wellness. What we have instead is
science-based, evidence-based strategies that scientists have shown will have some Wellness
value for us. We want evidence not hearsay.
At Wellness RISK Management we are structuring a new non-therapeutic, online, measurement
protocol. The first four pages of the measurement program are the measurements that create your Action
Plan List. These Action Plan items are things you are supposed to be doing and thinking on a
regular basis in your life. The next seven pages measure Self-Help clinical indicators for clinicians
and also measure you for indicators of Dr. Ouellette's Pain Relief Diet. That is because there is a FoodPainConnection that we like to
identify so we can come to some idea of how food may be affecting you.
At Startup on Your Initial Measurement
When you first take the questionnaire, you will have a Wellness RISK Management Instructor walk you through all 11 pages that create a
baseline of your 38 scores. Thereafter, when you take the measurement again, you can opt to complete only the
first 4 pages and the first item at the top of every page after that. At the end just click the
Submit Button at the bottom of the last page to send your results to Wellness RISK Management for processing into a Progress Chart and a ~35 page
Report of Findings for you. This will speed up the time it takes you to get your new Action Plan
List items. If you ever wish to check on your clinical scores again, then you can just complete the
entire questionnaire again.
You will receive two copies of your 38 scores called your Wellness
RISK Progress Chart. The first copy will have the file
codename you use and that codename will appear near the top of the Progress
Chart file. Keep that copy safe and do not give it to anyone so as to keep your secret codename
secret.
The second copy of your Progress Chart will be a redacted copy with the
codename removed from the file name and removed at the top of the progress chart. This is the copy you will give to
healthcare professionals, family and friends, if you wish to. You should ask them to place your name in the space
at the top of the page. This procedure protects your privacy as only you should know what your codename is. Never
give your Report of Findings to anyone.
Science is what directs us. Your job is to comply with the science as best you can and use
Wellness RISK Management Instructors to help yourself improve what you cannot improve
on your own.
The Average Person
For the average person the hardest part about lowering Wellness RISK is consistency. That is why Tutoring Workshops are so
important. Addressing all of Dr. Ouellette’s Five Pillars Optimal Health Secrets is a key point when
building overall Wellness. A person can take their time, however, they should expect to address all Five
Pillars every month in a consistently repeatable fashion. Wellness
RISK Instructors help with that.
Avoiding the very common inflammatory foods is top of the line in the Nutrition Pillar. Again
consistency is key. High intensity INTERVAL workouts and muscle building sports are top of the line in the
Exercise Pillar. You don't have to work into the exercise-pain-threshold to get results. So, know
your limits, know your injuries, know your age, know when you are sick. Again, consistency is key to successful
results. You can do it all on your own. Most people cannot. We help with that.
Spinal Health is about knowing your posture in everyday life, during workouts and during
Rest & Relaxation. These are two more of the Pillars. Again consistency is key. A positive
Attitude & Perspective with a willingness to get at 'getting the work done', rounds out
the Five Pillars.
Putting It All Together
To put it all together, getting measured for your Wellness RISK scores is the start, then connecting with a Wellness RISK Management Tutoring Workshop Instructor
is the way to learn how to measure yourself online and become consistent in your behaviours.
